How to Complete This Assessment

This assessment is completed by the supervisor or IPC Lead, not the staff member. For each topic, score the staff member on two measures:

Knowledge – Can the staff member explain the concept when asked? Score based on their verbal response or written answer.

Competency – Can the staff member demonstrate the skill in practice? Score based on direct observation during normal work tasks, a practical demonstration, or a simulated scenario.

The system uses the lower of the two scores to determine the training priority for each topic. A staff member who knows the theory but cannot perform the task still needs training.

1 = No knowledge or skill
2 = Limited understanding
3 = Developing
4 = Competent
5 = Expert / can teach others
